Pet Travel Certificates

Traveling with a pet? We’ve got you covered. Center for Veterinary Care Millbrook, P.C. has USDA Accredited Veterinarians to examine your pet and complete the necessary travel documents.

Here’s what you need to do:

We recommend you consult the USDA Animal and Plant Health Inspection Service Pet Travel site for important information before requesting an international health certificate. This site allows you to search by the destination country and will take you through the steps to obtain a Veterinary Export Health Certificate.

Know the time frame and requirements for your destination country as well as your airline prior to booking your travel. They each may have requirements regarding when you will need to have the health certificate issued. There may also be requirements for your destination regarding microchipping and vaccinations (especially Rabies) that will impact your departure date.

Contact Center for Veterinary Millbrook, P.C. to let us know that you need a pet travel certificate, your destination country, and your travel date. Complete all appropriate forms with all necessary information and a member of our team will contact you with any requests for clarifications. This must be done within a certain time frame before your travel date to ensure that all requirements for your destination country can be met. For New Clients, this will need to be booked at least 1 month in advance and all appropriate patient history and paperwork submitted to our team.

 

It is your responsibility to ensure you have the appropriate paperwork for your pet’s travel. You should check with both the airline and the consulate of the country you are traveling to. Specifically requesting import documentation. Also ensure that you have the appropriate travel container for your pet and whether your pet can travel in the cabin or is required to go as cargo. Please double check all paperwork and sign the appropriate sections. Please make copies and store separately. APHIS is a government agency, once paperwork is submitted it is completely out of our control. We will notify you when it is approved. We have no way to update you as to the return date, nor can we expedite any submission.

If you would like to overnight your paperwork to the USDA office (not recommended as overnight delivery is slower than E-submission), this form will be needed for endorsement fees.

Please complete this form and submit prior to your scheduled appointment.

Starting August 1, 2024, the CDC Dog Import Form is required to bring your dog back into the U.S.


This form should be completed by the person importing (bringing) the dog into the United States (the importer, owner, or shipper). After you submit this form, a receipt will be sent to the email address you provided. Please check that the email address is correct before submitting the form.
